Automating lead capture is the most effective way to grow your clinic without increasing front-desk workload. By embedding the PTWorkshops.com Appointment Request Form directly onto your website, you ensure that every prospect is immediately entered into our automated nurture system.


You can embed any of our Appointment Request or Funnel Landing pages directly on any website that accepts HTML.


Landing page links are dynamic, so if a workshop is no longer running, the link will lead to an appointment request landing page instead. Anyone signing up for a workshop using the embedded link on the same day as that workshop will be shown the appointment request landing page.

How to Find Your Landing Pages

  1. Log in to Breakthrough and click on the Settings gear at the bottom left.
  2. Click on Preview Links
  3. Locate the Appointment Requests section at the top of the page.
  4. Under the Embed Code header, click Copy to save the HTML snippet to your clipboard.
  5. Pro-Tip: For the best tracking, edit the src attribute within the code to include &utm_source=Website before pasting it onto your site. This ensures you can see exactly which leads came from your web form.

How to Embed the Form (By Platform)


While every website builder is different, use the following guide to find where to paste your code:

  • WordPress: Use a "Custom HTML" block. If using the Classic Editor, switch to the "Text" tab (not Visual).

  • Squarespace: Add a "Code" block to your section. Ensure the "Display Source" setting is unchecked.

  • Wix: Go to Add (+) > Embed Code > Embed a Widget and paste into the HTML Settings box.

  • GoDaddy: Look for the "HTML Code" section under the "Files & Web" or "Overlays" category.

Important: Most platforms require you to click "Publish" or "Update" after saving for the form to actually appear to your patients.



The Lead Journey & Automation


Once a patient submits the form, PTWorkshops.com triggers the following:

  • Immediate Tracking: The lead is added to your Contacts > Leads list.

  • Team Notification: A new task appears in your Follow-Up Tasks under Appointment Requests.

Automated Nurture: The lead is automatically enrolled in the "Appointment - Generic" campaign, receiving an immediate SMS and email to move them toward a scheduled evaluation.
