The /schedule Appointment Request page images have been updated

As requested by several of our customer, we've replaced the previous image of the older couple with an agnostic high resolution image of a general practice.

Bug Fix

[Practice.Name] no longer appears in the Text Message Body when the user presses the delete key when the field is empty.
